To characterize structural changes of HCQ retinopathy with SD-OCT after drug cessation. Setting: Private practice and academic institution. Patient Population: Patients at New England Eye Center and Ophthalmic Consultants of Boston in Boston, MA diagnosed with HCQ retinopathy and followed after drug cessation. Plaquenil somnolence The drug plaquenil 400 mg hydroxychloroquine Hydroxychloroquine is a well-tolerated medication for various rheumatologic and dermatologic conditions. Its main side effects are gastrointestinal upset, skin rash, headache, and ocular toxicity1. Within the eye, hydroxychloroquine can adversely impact the cornea, ciliary body, and retina1 Bull's-eye maculopathy due to hydroxychloroquine toxicity. Contributor Jesse Vislisel, MD Photographer Randy Verdick, FOPS September 9, 2013. This patient took hydroxychloroquine for rheumatoid arthritis and her vision dropped to 20/300 as a result of the retinal toxicity. Hence, it is imperative that we become familiar with recognizing the spectrum of HVF and SD-OCT findings associated with Plaquenil retinal toxicity. Of note, SD-OCT, in combination with Humphrey visual field testing, is critical for the early detection of Plaquenil retinal toxicity. Main Outcome Measures: SD-OCT findings suggestive of HCQ retinopathy before parafoveal ellipsoid disruption. Retrospective clinical data review by the Boston Image Reading Center. Patients with renal or hepatic dysfunction are at an increased risk of toxicity. 1 Other risk factors for retinal toxicity include short stature, obesity, advanced age and/or a pre-existing macular pathology. 1 So, what does this mean for our patient? Are the retinal changes a result of Plaquenil use?
